Scary but true stories

By now the stories associated with the BP refinery explosion in Texas City in 2005 are becoming part of industrial legend. Thanks to your tax dollars, you can now get amazingly detailed accounts of the accident, its aftermath, and perhaps most interesting, its causes.

The Chemical Safety Board has released a huge amount of information compiled in its investigation, apparently with the cooperation of BP, which talks about a whole series of failures that led to the explosion with all its injuries and deaths.  Click here to go to the CSB Website where you can look at all sorts of documents, if you want to study this cautionary tale.
